PHARMACOLOGICAL ACTION Hypoglycemic drug, selective inhibitor of dipeptidyl peptidase-4 (DPP-4). Rapid and complete inhibition of DPP-4 activity causes an increase in both basal and meal-stimulated secretion of glucagon-like peptide type 1 (GLP-1) and glucose-dependent insulinotropic polypeptide (GIP) from the intestine into the systemic circulation throughout the day. By increasing the levels of GLP-1 and GIP, vildagliptin causes an increase in the sensitivity of pancreatic beta cells to glucose, which leads to an improvement in glucose-dependent insulin secretion. Vildagliptin improves pancreatic beta cell function. By increasing endogenous GLP-1 levels, vildagliptin increases alpha cell sensitivity to glucose, resulting in improved glucose-dependent regulation of glucagon secretion. Reducing the level of excess glucagon during meals causes a decrease in insulin resistance.
