Flucinar ointment, like the gel, has antiexudative, antipruritic, antiallergic and anti-inflammatory effects.
Its effect is achieved by inhibiting phospholipase A2, reducing PG synthesis and releasing macrophage chemotactic factor.
This ensures the migration of macrophages and lymphocytes to the area of inflammation, stabilization of the lysosome membrane, prevention of the release of lysosomal enzymes , blocking the release of biologically active components from sensitized mast cells, histamine basophils, and so on.

Interactions of the drug Flucinar
No interactions were observed with external use of GCS. However, it should be noted that during treatment with the drug it is not recommended to vaccinate against smallpox, as well as carry out other types of immunization (especially with prolonged use on large areas of the skin) due to the possible lack of an adequate immunological response in the form of the production of appropriate antibodies. Flucinar may enhance the effect of immunosuppressive drugs and weaken the effect of immunostimulating drugs.

Special instructions for the use of the drug Flucinar
Do not use continuously for more than 2 weeks. With prolonged use on large areas of the skin, the incidence of side effects and the possibility of developing edema, arterial hypertension, hyperglycemia, and decreased body resistance increases. With external use of the drug, the following are possible: a decrease in the production of ACTH by the pituitary gland due to inhibition of the adrenal-pituitary gland system, a decrease in the level of cortisol in the blood and the development of Cushing's syndrome, which disappears after stopping the use of the drug. If an infection develops at the site of application of the ointment, appropriate antibacterial or antifungal treatment should be carried out. If symptoms of infection do not disappear, you must stop using the drug while the infection is being treated. Avoid applying the drug to the eyelids or the skin around the eyes in patients with angle-closure and open-angle glaucoma, as well as in patients with cataracts due to a possible increase in the symptoms of the disease. On the skin of the face, axillary and groin area, use only in case of special need, since there may be increased absorption and a high risk of side effects (telangiectasia, perioral dermatitis), even after short-term use. The drug should be used with caution in the presence of subcutaneous tissue atrophy, mainly in the elderly. Use during pregnancy and lactation. As a result of animal studies, it has been confirmed that GCS have a teratogenic effect even when used in small doses. The teratogenic effect in animals was also confirmed when strong corticosteroids were applied to the skin. Controlled studies have not been conducted to evaluate the possible teratogenic effects in women during pregnancy when applied topically to the skin. Flucinar can be prescribed during pregnancy only if the potential benefit of the drug for the mother outweighs the risk to the fetus. The drug is absolutely contraindicated in the first trimester of pregnancy. It is not known whether fluocinolone acetonide can pass into breast milk after topical use. After oral administration of corticosteroids, which can potentially cause effects in a newborn baby, their significant concentrations in breast milk have not been determined. The antiallergic effect is manifested by inhibiting the development of local hypersensitivity reactions. As a result of local vasoconstrictive action, it reduces the risk of developing exudative reactions. Reduces protein synthesis and collagen deposition. Accelerates the breakdown of proteins in the skin and weakens proliferative processes. Pharmacokinetics. Fluocinolone acetonide penetrates well through the stratum corneum of the epidermis, where it gradually accumulates and is detected even 15 days after application. Does not biotransform in the skin. After absorption, it is systemically biotransformed, mainly in the liver. It is excreted in the urine and in smaller quantities in bile, mainly in the form of a compound with glucuronic acid, and also in a small amount unchanged. Absorption of fluocinolone acetonide through the skin is enhanced when applied to sensitive skin - areas of anatomical folds of the body and face, as well as to skin with damaged epidermis or inflammation. The use of an occlusive dressing causes an increase in skin temperature and humidity, which also leads to increased absorption of fluocinolone acetonide. In addition, absorption increases with frequent use of the drug and over large areas of the skin. Absorption through the skin occurs faster in adolescents than in adults.
