Compound

The Raunatin tablet, coated with a special coating, contains 2 mg. active compound of rauwolfia alkaloid , as well as auxiliary compounds: cerulesum, sugar, calcium stearate, cross-povidone, glucose, stearic acid, basic magnesium carbonate, gelatin, aerosil, medium molecular weight polyvinylpyrrolidone , talc, pigment titanium dioxide, beeswax, dyes E 104 and KF-6001.
The drug belongs to the group of sympatholytics , as well as antihypertensive drugs. The question of what Raunatin tablets are for can be answered as follows - the drug is indicated for hypertension .

Side effects

The drug is usually well tolerated. Sometimes reactions that are listed below.

From the cardiovascular system: edema, hot flashes, bradycardia; in patients with angina, increased pain in the heart area is possible; with prolonged use, an increase in the frequency of angina attacks is possible.

From the nervous system: headache, drowsiness, sweating, depression, general weakness, with prolonged use - the appearance of nightmares, the development of symptoms of parkinsonism.

From the organ of vision: miosis.

From the digestive system: nausea, vomiting, diarrhea, abdominal pain, increased or decreased appetite, drooling, dry mouth, increased secretion of gastric juice.

From the digestive system: with long-term use in high doses, liver function may be impaired.

From the genitourinary system: dysuria, impotence.

From the endocrine system and metabolism: decreased libido, sodium retention, increased secretion of prolactin, galactorrhea, gynecomastia, engorgement of the mammary glands, weight gain.

Allergic reactions: skin rashes, itching, dry mucous membranes, swelling of the nasal mucosa.

From the hematopoietic system: thrombocytopenic purpura.

Side effects disappear when the dose is reduced or after a short-term (1-3 days) break in taking the drug.

Pharmacodynamics and pharmacokinetics

Since Raunatin contains components such as rauwolfia alkaloids , it has a pronounced hypotensive as well as antiarrhythmic effect . The drug has a beneficial effect on the health of patients suffering from arterial hypertension , both mild and moderate.

It is noteworthy that the sedative effect of this drug is less pronounced than its antiarrhythmic and hypotensive properties . However, in comparison with Reserpine, Raunatin is in most cases much better tolerated, although the pronounced hypotensive effect of this drug occurs more slowly.

Features of application

Pregnant

The use of the drug during pregnancy and breastfeeding is contraindicated, since the alkaloid reserpine penetrates the placenta and into breast milk. If it is necessary to use the drug during treatment, you should stop breastfeeding.

Children

There are no data on the use of the drug in children.

The drug may impair the patient's ability to react quickly, especially at the beginning of treatment, as a result of which, during the course of treatment, patients should avoid potentially hazardous activities that require increased attention and speed of psychomotor reactions.

Contraindications

It is not recommended to use the drug for:

  • depression;
  • organic myocardial damage ;
  • arterial hypotension;
  • the patient's age is under 18 years;
  • coronary circulatory insufficiency ;
  • aortic defects;
  • pregnancy (third trimester);
  • nephrosclerosis;
  • stomach and duodenal ulcers;
  • hypersensitivity.

Overdose

Symptoms: sweating, general weakness and severe arterial hypotension. In severe cases, after a short period of euphoria, drowsiness, physical inactivity, severe depression (which can lead to suicide), severe parkinsonism, loss of consciousness, coma, convulsions, respiratory depression, and hypothermia develop.

Treatment: gastric lavage, taking activated charcoal. Forced diuresis is indicated. In case of a pronounced decrease in blood pressure, in case of collapse, the patient should be placed and his legs raised, if necessary, intravenous administration of rheopolyglucin or mezaton with the addition of angiotensinamide or a 2% solution of norepinephrine hydrogen tartrate is indicated. Sympathomimetic drugs should be prescribed with great caution due to the risk of developing pulmonary edema. Inject a 10% solution of caffeine sodium benzoate subcutaneously. The presence of severe respiratory depression or its delay requires suction of mucus from the respiratory tract, intubation and artificial ventilation, oxygen therapy.

There are no specific antidotes for poisoning with raunatin and other rauwolfia preparations.

Raunatin tablets, instructions for use (Method and dosage)

In accordance with the instructions for use of Raunatin, tablets are taken after meals. In order not to harm the patient’s health, the patient should strictly adhere to the drug dosage regimen, and, in addition, the recommended dosages. On the first day, take one tablet, i.e. 2 mg each. rauwolfia alkaloids before bed.

On the second day - one tablet twice a day, the next day - three tablets, thus gradually increasing the dosage to six pieces. After a two-week therapeutic course of treatment, the dosage is also gradually reduced to a maximum of two tablets per day.

  • Active component: Raunatin (Rauwolfia alkaloids) in terms of 100% content of the total alkaloids - 0.002 g;
  • Kernel excipients: Sugar (sucrose) - 0.0346 g; Potato starch - 0.0290 g; Glucose monohydrate (dextrose monohydrate) - 0.0136 g; Calcium stearate - 0.0008 g; Kernel mass - 0.08 g;

The tablets are round, biconvex, white coated.

Directions for use and doses

Inside, after eating. Treatment should be carried out under the supervision of a physician.

On the first day, take 1 tablet (0.002 g) at night; on the second day - 1 tablet 2 times a day; on the third day - 3 tablets.

The maximum daily dose is 6 tablets per day. After achieving a therapeutic effect (after 10-14 days), the dose is gradually reduced to 1-2 tablets per day. Maintenance dose - 1 tablet per day.

The course of treatment is 3-4 weeks, repeated courses are recommended by a doctor.

Pharmacodynamics

Raunatin is the sum of alkaloids from rauwolfia roots, the content of which in terms of dry matter is at least 90%. Alkaloids are represented by reserpine, serpentine, ajmaline and others.

It has a hypotensive effect and antiarrhythmic effect, as well as a calming effect on the central nervous system. The latter is less pronounced than that of reserpine, but the drug is not inferior to reserpine in its hypotensive effect. In some cases, Raunatin is better tolerated than reserpine. The hypotensive effect of the drug develops more slowly than that of reserpine.

Pharmacokinetics

It is well absorbed from the gastrointestinal tract, but more slowly than reserpine, so the hypotensive effect develops gradually. Approximately 60% of the drug is excreted in feces and 8% in urine. The half-life is 45-168 hours.

Contraindications

Hypersensitivity to the components of the drug, arterial hypotension, severe bradycardia, coronary circulatory failure, organic myocardial lesions, aortic defects, gastric and duodenal ulcers, nephrosclerosis, depression, sucrase/isomaltase deficiency, congenital fructose intolerance, glucose-galactose malabsorption, pregnancy, period of breastfeeding, age up to 18 years.

Carefully:

Cardiac ischemia; patients with diabetes and people on a low-carbohydrate diet.

In patients with coronary heart disease, an increase in angina attacks is possible. If pain in the heart area occurs, it is necessary to reduce the dose of Raunatin or interrupt treatment (for 1-3 days) and consult a doctor.

Before planned surgery, the drug should be discontinued several days before surgery. If emergency surgery is necessary under general anesthesia in patients taking Raunatin, premedication with atropine is necessary.

The drug should not be used before an electropulse therapy session. The dose of the drug should be reduced gradually.

Patients with diabetes should take into account that one tablet contains about 0.139 g of carbohydrates (0.012 XE).

Impact on the ability to drive vehicles and operate machinery

During treatment with the drug, patients should refrain from driving vehicles and engaging in other potentially hazardous activities that require increased concentration and speed of psychomotor reactions.

Overdose

Symptoms: pronounced decrease in blood pressure up to collapse, increased sweating, general weakness.

Treatment: symptomatic - gastric lavage, taking activated charcoal. If there is a pronounced decrease in blood pressure, if the patient collapses, lay the patient down and raise his legs; if necessary, intravenous administration of rheopolyglucin or phenylephrine solution or other vasoconstrictors is indicated. There are no specific antidotes.

Possible allergic reactions, swelling of the nasal mucosa, increased sweating, asthenia, bradycardia, pain in the heart, worsening angina, development of depression or worsening of its course, decreased libido.

Drug interactions

Can be used simultaneously or sequentially with other antihypertensive drugs (ganglion blockers, hydrochlorothiazide, hydralazine and others).

Prescribe with caution in combination with quinidine, guanethidine, cardiac glycosides, beta-blockers (risk of increased severity of negative chrono- and dromotropic effects).

The drug can enhance the effect of ethanol and barbiturates on the central nervous system.
