The drug Teraligen belongs to the group of antipsychotics or neuroleptics. Sometimes its other name is teraligen valence. The most common form of the drug is tablets. They are recognizable by their dark pink film shell. The active substance in the medicine is alimemazine tartrate in an amount of 5 mg. Auxiliary ingredients used in production are listed in the instructions for use. You should familiarize yourself with the composition of the drug to exclude the occurrence of allergic reactions during treatment.

Pharmacodynamics and pharmacokinetics

Teraligen is an antipsychotic (antipsychotic) that has serotonin-blocking , antispasmodic and antihistamine effects, as well as hypnotic , antiemetic , antitussive and sedative effects.

You can observe the effect after taking it within 10-15 minutes, its duration of action is from 6 to 8 hours.

The drug has low antipsychotic activity , so it is ineffective in the acute stage of psychotic conditions .

It is also used for the treatment of elderly people, as well as children and adolescents, as the drug is well tolerated.

Absorbed completely, quickly, the maximum concentration in plasma can be observed after administration after 1-2 hours. Within 2 days it is excreted as a metabolite by the kidneys (70-80%).

Results and discussion

The most common complaints among patients were general anxiety, tearfulness, fear for their health, sleep disturbances, frequent headaches, increased fatigue, decreased performance, pain and discomfort in the heart, digestive disorders (unstable stool, pain and discomfort in the abdomen). Also, when filling out a questionnaire to identify vegetative changes, most patients noted a tendency to redness (less often, paleness) of the face when excited, difficulty breathing, poor tolerance of stuffy rooms, and episodes of fainting.

When studying cognitive functions according to the MMSE, the most characteristic disorders were errors associated with increased impulsivity or, conversely, decreased initiative, lack of attention, and difficulty following a given program. When assessing the results of learning 10 words (according to A.R. Luria), all patients had a normal indicator of delayed reproduction of memorized words (at least 5). However, during the learning process, approximately 1/3 of the subjects showed increased exhaustion, when, with repeated presentations, the number of words reproduced began to decrease.

Treatment was generally well tolerated. The most common problem was drowsiness during the first days of taking the drug.

When assessing the condition of patients on the 30th day from the start of treatment, most of them noted positive dynamics. First of all, this concerned improving the quality of sleep: easier falling asleep and deeper sleep during the night. Patients with cephalgic syndrome paid attention to a decrease in the frequency and intensity of headaches. Also, many patients noted that they began to react more calmly in the event of stressful situations.

On the 60th day, almost all patients noted a significant improvement in sleep quality, a decrease in anxiety, pain syndromes, as well as an improvement in general well-being and increased activity.

Analysis of the dynamics of autonomic disorders using a questionnaire to identify autonomic changes showed a significant decrease in their severity during treatment. At the same time, in the control group there were no pronounced dynamics of vegetative symptoms, but there was a slight tendency towards their increase (Fig. 1).

In the group receiving treatment with teraligen, the rates of ST and LT decreased significantly both on the 30th and 60th days of treatment.

In the control group, contradictory dynamics were noted: on the one hand, CT tended to decrease, but at the same time, a significant increase in the RT indicator was noted (Fig. 2).

Also, a significant decrease in anxiety and depression scores during treatment with Teraligen was noted according to HADS. In the control group there were no significant changes, but, on the contrary, there was a tendency towards a gradual increase in indicators (Fig. 3).

Regarding cognitive functions, most patients noted an improvement in the ability to concentrate and, as a result, an improvement in memory and performance.

When comparing data from a study of cognitive evoked potentials at the first and final visits, a statistically significant decrease in the P300 latent time was noted in the group receiving treatment with teraligen ( p

<0.05), which indicates positive dynamics.
On the contrary, in the control group a statistically significant decrease in P300 amplitude was detected ( p
<0.05), which may indicate a deterioration in cognitive functions.

Despite the fact that the data from the P300 study, in general, confirm the results of neuropsychological testing, it should be noted that the results of the P300 study should not be taken as absolutes, since a feature of cognitive evoked potentials is high variability depending on age, and in our study there was a fairly significant scatter on this basis.

Contraindications

Teraligen has a number of contraindications:

Teraligen and alcohol

Instructions for use of Teraligen (Method and dosage)

The tablets are intended for oral administration.

To obtain a hypnotic effect, adults need to take 5-10 mg per day, an anxiolytic effect - 60-80 mg, and in a psychotic state - 0.2-0.4 g.

Instructions for Tiralijen for children (over 7 years old): to achieve a hypnotic effect - 2.5-5 mg, anxiolytic effect - 20-40 mg, for the treatment of allergic reactions - 5-20 mg. The daily dose for a psychotic state can be increased to 60 mg.

The daily dose should be divided into 3-4 doses.

Interaction

The drug can enhance the effect of anxiolytics , hypnotics , antipsychotics , narcotic analgesics , general anesthesia , antihypertensive drugs and m-anticholinergic drugs . In such cases, it is necessary to adjust the dose.

Teraligen reduces the effectiveness of m-cholinergic stimulants , dopamine , ephedrine , amphetamine , levodopa , guanethidine . Significant depression of the central nervous system is observed when taken simultaneously with drugs that suppress central nervous system functions and ethanol . Combination with barbiturates and antiepileptic drugs leads to a decrease in the threshold of convulsive readiness.

An increase in the plasma concentration of alimemazine is caused by beta-blockers , which can lead to the development of arrhythmia and a decrease in blood pressure.

Teraligen increases the level of prolactin in the blood and reduces the effectiveness of bromocriptine .

The M-anticholinergic activity of alimemazine increases while taking anticholinergics and tricyclic antidepressants .

The risk of extrapyramidal disorders and arterial hypotension increases when taking MAO inhibitors and phenothiazine derivatives .
