For sore throat: lozenges, tablets and lozenges (in tables and figures)

Pain-relieving lozenges, tablets and lozenges are in constant demand, because discomfort in the throat and mouth is one of the most common symptoms of colds and acute respiratory viral infections. At the same time, most drugs from this group are sold without a doctor’s prescription, which means that first-generation companies have a real opportunity to help the client with their choice and increase sales. The success of the consultation largely depends on how well pharmacists know about numerous antiseptics for sore throats. In our tables, we have compactly and clearly provided maximum data on the properties of drugs in this group.

All local drugs for the treatment of sore throat can be divided into monocomponent (see point 1 of this review) and multicomponent (point 2). Among the latter, it is worth highlighting bacterial lysates (Table 3), which belong to the group of immunomodulators and act by increasing the number of immunocompetent cells, increasing the production of lysozyme, interferon and secretory immunoglobulin A in saliva. The composition of bacterial lysates corresponds to the pathogens that most often cause inflammatory processes in the oral cavity and pharynx.

The vast majority of local drugs for sore throat have a combined composition, which may include antiseptic components (this is the basis of almost any drug), antibiotics, anesthetics, less often - NSAIDs, as well as other components, for example, essential oils.

Action

Sage is known primarily for its active participation in the fight against microorganisms and inflammatory processes. Cineole is a component of sage essential oil, has phytoncidal properties and is involved in the neutralization of bacteria. Sage also has antiseptic properties due to the presence of salvin, a plant antibiotic. In addition, the use of sage extract helps reduce inflammation and alleviate discomfort in the throat. It contains so-called tannins.

Thanks to them, the mucous membranes are protected from irritation and further damage. The presence of flavonoids in the composition helps to reduce the permeability of cell membranes, walls of blood and lymphatic vessels.

Marshmallow contains specific substances - polysaccharides, due to which it is used as a remedy that helps reduce inflammation, relieve cough, and stimulate protective immune mechanisms.

Plantain has antiseptic and antimicrobial properties. In addition, it helps reduce the allergic reaction to infectious agents and thin the sputum. Participates in tissue regeneration and restoration of the mucous membranes of the respiratory tract due to its anti-inflammatory properties.

Vitamin C (ascorbic acid) is an antioxidant that protects cell membranes from the damaging effects of free radicals during the development of inflammation. A sufficient amount of vitamin C in the body is necessary to provide effective immune protection against infections. It stimulates the functioning of the immune system at the cellular level, activating specific cells that are the main assistants in the healing process. Ascorbic acid also acts as an antiviral component that increases the amount of interferon. Vitamin C is not synthesized in the body, so it must come from outside.

Menthol acts on cold receptors on the surface of the mucous membrane of the throat, promotes a local decrease in sensitivity, facilitates swallowing and reduces the severity of discomfort in the throat.

Use during pregnancy and breastfeeding

Contraindicated during pregnancy and lactation.

Release forms and indications

The vast majority of drugs for sore throat are available in the form of lozenges or lozenges. With the exception of three drugs.

A drugRelease form
Gramicidin SCheek tablets
IsmigenSublingual tablets
Falimint*Dragee

All local medications for sore throat are used for infectious and inflammatory diseases of the oral cavity and pharynx. Bacterial lysates are also indicated for the prevention of exacerbations of recurrent infections (Imudon - for the prevention of diseases of the oral cavity and pharynx, and Ismigen is also used for the prevention of lower respiratory tract infections). And “Falimint” has another “non-standard” indication for drugs for sore throat: the drug is prescribed to relieve a reflex non-productive cough (for example, a smoker’s cough).

pharmachologic effect

Sage has an antimicrobial effect, which is enhanced by eucalyptus oil.

Licorice stimulates the activity of the ciliated epithelium and enhances the secretory function of the mucous membranes of the upper respiratory tract, which contributes to the dilution of sputum and its rapid evacuation.

A specially selected substance - cetylpyridine chloride - destroys infectious agents.

Menthol cools and has a mild local anesthetic effect. Ascorbic acid mobilizes the body's defenses to fight infection, and also normalizes capillary permeability, which is often impaired during inflammatory processes.

* Allowed for diabetes.

The vast majority of local remedies for sore throats contain sugar, but there are still preparations with sweeteners, which differ in the size of the glycemic index (GI), i.e., in the effect on blood glucose levels, and in calorie content. Among all the sugar substitutes used in the group of drugs under review, only one, sucralose, is characterized by zero GI and calorie content. Aspartame, sorbitol, and mannitol also have a GI that does not exceed zero, but they contain several kilocalories per gram (1.4, 3.5, and 9, respectively). Patients with diabetes should take medications containing maltitol with caution: its GI is 25–35 units, and its calorie content reaches 13 kilocalories per gram. For comparison, the GI of milk sugar, which is most often used as a sweetener, reaches 46 (calorie content 16 kcal), and glucose - 100 units with a calorie content of 16 kcal/1 gram.
